Bootham Park Hospital occupies extensive open grounds extending to around 10 acres to the south, between the hospital and Bootham. This open space can be identified in the York Central Historic Core Conservation Area appraisal as being a significant feature of the Bootham Park Hospital character area. Within the site to the east are additional green spaces surrounding the Chapel.

The principal hospital buildings on site are Grade I & Grade II Listed and include a complex group of structures of various states and styles. The three storey frontage of the original 1777 structure is by the celebrated architect John Carr and is Grade I Listed. To the rear of this building a T plan range dates to the late 18th and 19th centuries with various modern additions.

Within the grounds there are various buildings of note including the former estate cottages which comprise a cluster of properties along The Avenue in the region of 7,395 sq ft GIA (687 sq m) and forms the boundary with Bootham School. To the entrance of the site, on Bootham, there is a Grade II listed gate lodge that dates from 1857 that was previously used as administration space, which comprises approximately 1,496 sq ft GIA (139 sq m).
